Telehealth Consent Notice

1. Nature of Telehealth Services

Optimum Counseling PLLC provides mental health services exclusively via telehealth — meaning all sessions are conducted remotely using secure, HIPAA-compliant video and/or audio technology. By scheduling and participating in services with our practice, you acknowledge and consent to the delivery of services in this format.

2. Benefits of Telehealth

Telehealth offers meaningful advantages, including:

  • Access to high-quality clinical care without geographic limitation

  • Flexibility and convenience for individuals with demanding schedules

  • Continuity of care across state lines for clients who travel or relocate

  • Enhanced privacy — no waiting rooms, no in-person visibility

3. Risks and Limitations

You should be aware of the following limitations associated with telehealth:

  • Technology failures (internet outages, platform disruptions) may interrupt sessions

  • Telehealth may not be appropriate for all clinical presentations or levels of care

  • The clinician's ability to observe nonverbal cues may be limited compared to in-person sessions

  • Confidentiality may be compromised if you participate from a non-private location

4. Your Responsibilities

To ensure the safety and effectiveness of telehealth services, you agree to:

  • Participate from a private, confidential location where you cannot be overheard

  • Use a stable internet connection and a device with functioning audio and video capabilities

  • Not record sessions without the express written consent of your clinician

  • Provide your current physical location at the start of each session for emergency purposes

5. Emergency Protocols

Telehealth is not appropriate for mental health emergencies. If you are in crisis during or between sessions, please call 911 or contact the 988 Suicide & Crisis Lifeline immediately. Your clinician will discuss an individualized safety plan with you as part of the intake process.

6. Multi-State Practice

Optimum Counseling PLLC is currently licensed to provide telehealth services in Texas, Arizona, Colorado, Washington, Vermont, Louisiana, and California. Services may only be provided to clients physically located in a state where our clinicians hold active licensure at the time of service. It is your responsibility to notify us if you relocate or plan to participate in sessions from a state not listed above.

7. Technology Platform

Sessions are conducted through a HIPAA-compliant telehealth platform. Login instructions and session links will be provided upon scheduling. If you experience technical difficulties, please contact our office as soon as possible.
